Planning your first visit to our practice? On this page, you’ll find the information you need to get started, such as a map and directions to our offices.

Patient Guide

We want to make your visit as pleasant as possible, and hope that the following information will be of help to you and assist in your care.

Please bring any previous EKG, chest X-ray and/or hospital summaries you may have.

Please arrive 15 minutes prior to your appointment time to complete the necessary check-in tasks, and be certain to bring the following to your appointment:

  • The name, address and telephone number of your primary care provider;
  • Your insurance card (needed at each visit);
  • Any required referral forms;
  • A photo ID;
  • An accurate list of medications you are currently taking;
  • If you are scheduled for testing, please follow the instructions given to you or downloaded from our website.

At your visit, a medical technician will greet you and bring you to an examination room. They will get your weight, blood pressure and usually an EKG before the doctor sees you. They will also confirm your current medications and some medical history.

An initial visit is scheduled for 45 minutes. A follow-up visit is usually scheduled for 15 minutes. Individual tests can range from 15 – 60 minutes depending on the test. Please bring any questions or concerns to discuss with the practitioner so that you can make the most of your time with him/her.

Referrals

As cardiologists, we serve as adjunct practitioners to your primary care provider. Your primary care provider will continue to serve your non-cardiac needs on an ongoing basis. We will keep your primary care provider up-to-date on your diagnosis, treatment and progress. Most insurance carriers require a written referral from your primary care provider. Please make sure that your primary care provider sends us your necessary referral in a timely manner. If we have not received a necessary referral prior to your visit, we will reschedule your appointment in order to give you more time to obtain the necessary referral.
